I see these spacers advertised everywhere. Are they effective? It looks as if the fuel lines would be too short, do they come with extensions? What about hood clearance with an aftermarket air cleaner?

Never mind, I searched here and got my answer, sort of. They help a TBI but not a port injected model. How much? Worth the cost and effort? It MIGHT work is the correct answer to your question. Trial is the only way to know. It is also the only way to know what thickness you should use.
What you need to determine is if the stock plenum volume is hurting performance. If so then you will see a gain, if not you will see a loss. The other effect the spacer will give is moving the peak torque up in the RPM range. An effect some will see as a drawback. Also when you change your cam, intake or exhaust the spacer needs to be re evaluated.
The best street test you can do is to install one and drive it and see if you feel a difference and then log your gas millage carefully. If you see a marginal improvement in millage then your engine is running more efficiently.1993 EXT. CAB, 3.4L V6 TBI, 5spd manual. Sonoma
