This is the 2.8 I engine that I am planning to replace with another 2.8 or 4.3. When tearing down, came across this little gem... Anyone have an idea how this happened. The push rod dropped and the tappet is missing pieces (see photos)

Looks like the lifter collapsed and just fell apart. Over rev'd, no oil?Lifting my front wheels, one jack at a time.
