Question : 94 Trans Issue

My Mom's 94 Grand Prix,when she puts the gear selector from reverse to drive,
sometimes won't drop into drive and move forward until she moves the gear selector in n outta park, then it works fine, it doesn't do it all the time. any help would be appreciated. Thanks, Mike

also I noticed this morning when it was cold the gear shifter would move but the orange gear selector was out of sync with the shifter. Thanks
