So how exactly do you connect pushrods to the brake pads?
Anyway, The issue is that the pad is worn out, not that the friction material is breaking way. At the point that the friction material is broken away in the pictures above, the pads are long past the point of needing to be replaced.
It's not an issue with just riveted ones. I had a set (I think from Autozone but don't quote me on that) that had started making an odd squeaking sound and a small section in the middle of the pad looked like it folded over. Almost as if there had been an air bubble inside the friction material. They had been installed for about three months before this occurred. However all the pads had worn evenly. If you notice, out of the four pieces pictured, only the damaged one has significant wear. I guess it could be a dud pad but I would also check out if there was any kind of binding with the caliper or damage to the rotor surface. The Unevenness of the wear on the on pad is more unusual than the fact that the material broke away.

The pad was on the outer side of the rotor face. When the piston in the caliper pushes the other side will pull. So both sides should have even pressure when braking and you are correct that the pad wearing faster is not right.
Anyways I changed the caliper when replacing the front brakes.
