Done it without tilting, however if you are on original plugs and wires and are going to tune up you may want to consider changing the upper and lower intake manifold gaskets anyway, which would make a extension combo even easier and if you chose to use a quality elbow I would make sure a feel comfortable using one and understand the dynamics because one wrong move can break a plug.

I used to park with the nose facing down our drive since it was sloped. I would leave it in gear and pull the bolt for the dogbone and let the weight of the car do the work to rotate the engine for me. When I was done, I had someone get in and put it in neutral with the brakes on so I could hook the dogbone back up.
